AFFIDAVIT OF HUSBAND/WIFE:

(l) “Disposition” means and refers to assets which have been sold, transferred, given away,
consumed, or exchanged for other property. It includes withdrawal of funds from accounts,
cashing in life insurance policies, rollovers or distributions of retirement benefits, and
obtaining loans against assets.

DISCLOSURE OF DOCUMENTS. You are required to serve on the opposing party with your
Affidavit copies of the following documents, to the extent they are in your possession or under your
control. DO NOT ATTACH THESE DOCUMENTS TO THE ORIGINAL AFFIDAVIT WHICH
IS FILED WITH THE COURT. In Part VI, you must list the documents you are providing.
