Non-resident Off-Highway Vehicle User Permit Application Instructions
With the exception of Off-Highway Vehicles (OHVs) registered in a state that offers reciprocal operating
privileges to Utah residents, Utah law requires that any non-resident owning an OHV who operates or gives
another person permission to operate the OHV on any public land, trail, street, or highway in Utah must pay
an annual OHV user fee. Revenues generated by OHV user fees are dedicated to the construction,
improvement, operation and maintenance of trails. Individuals wanting to obtain a non-resident
OHV user permit must provide proof of residency in a state other than Utah, and must provide proof that
the OHV is not owned by a Utah resident.
Please Note: This permit is valid for ONE (1) year from the month of purchase.
The process for obtaining a non-resident OHV user permit by mail is outlined below.
(NOTE: Incomplete applications, or applications submitted on anything other than the approved form, or
application packets which do not contain all required fees and documents will not be processed.) Permits
are also available in person at authorized vendor locations throughout the state of Utah. For a complete

1. Complete a Non-resident Off-Highway Vehicle User Fee Application and Affidavit for each OHV you will
be bringing to Utah. Have the affidavit(s) notarized by a Notary Public.
2. Attach a photocopy of your state issued identification (Driver license or other state issued identification)
to the affidavit.
3. Attach a photocopy of each OHV title, current registration, or bill of sale to the affidavit.
4. Include check, money order or credit card information in the amount of $30 dollars ( US) for each OHV.
5. Mail or Email all the above documents to:
