This form is designed to be used for supervision/administration of routine medication. It should be kept with the relevant medication authority for each separate medication on the site. Medication logs can

be taken on excursions together with the appropriate medication and returned to the agreed storage place after the event.

NOTE: Medication logs are formal records and logs no longer in use need to be stored in the site’s child/student record folder for future reference.
