CONSENT & LIABILITY RELEASE

By signing this form, you give the Cool Kids Campaign permission to share your child’s name and photos as it relates to

her involvement in the American Girl Fashion Show® Model Search and, if selected, the official Fashion Show on May 2

and 3, 2015.

You also allow us to use and distribute interviews, photography, videotapes and audiotapes of your child for public relations,

advertising and web communications purposes for current and future promotion of the American Girl Fashion Show®.

By signing the above application, I also agree to have my child participate in all mandatory requirements, including

fittings and photographs, rehearsals, and a dress rehearsal. In addition, I understand that my daughter will be asked

to raise pledges in keeping with the charitable aspect of the event.

I release and hold harmless Cool Kids Campaign Foundation, American Girl Programs, and any other sponsors, volun-

teers, or staff for said event from any and all liability that may arise from my daughter’s participation in the meetings,

rehearsals or fashion show described above.

This authorization is valid for two years from the date it is signed, unless you withdraw your permission sooner by

notifying us in writing at the address below:
