SUBLET AGREEMENT

Note: If this sublet form & $150 dollars is not completed or paid and a sublet is found to be staying unauthorized

at your unit, JDN will serve the unknown tenant with a “form A2” unauthorized occupant”. This fee can be

passed on or shared with the new tenant.

Whereas the Tenants are signatories to a certain Tenancy Agreement dated

(Landlord)

and whereas the Tenants are desirous of subletting their premises and whereas the Sub-tenants are

desirous of occupying the premises now currently rented by the Tenants, municipally known as

___________________________________(address) London, Ontario and the parties hereto agree as

follows:

The Sub-tenants' tenancy shall commence on ____ day of ________ 20_____ and shall be

complete and ended on _______ day of _______20______. (end date of the original lease)

The Sub-tenants shall pay to the Tenants the sum of _____________ per month for each

month in the above term.

The Tenants will direct the Sub-tenants to pay the aforementioned rent directly to the

Tenant or to whomever the Tenants direct. The Tenant will remain paying the Landlord.

The Sub-tenants shall abide by all terms and conditions in the Tenancy Agreement dated
