Owner’s inquiries about Pet, notify Owner of changes of address and telephone number of Caregiver, and

return Pet to Owner upon request. As used in this Agreement, “Care” shall include providing Pet with food,

water and shelter, giving reasonable attention to Pet’s health, safety and grooming, and carrying out the

following special instructions:

Except as provide d ab ove, Caregive r will have no liability to Owner except that which might

arise irresp ective of this A gree m ent.

Owner authorizes Caregive r to se ek app ropriate veterinary medical treatment or attention on

behalf of Pet as may be reasonably required by the circumstances, including but not limited to examination,

preventative and/or curative treatment, x-ray, laboratory examination, anesthetic, veterinary medicine

diagnosis, and consultations (“Veterinary Services”). Pet shall not be subjected to research or

exp erim enta tion.

Owner authorizes Caregiver to mak e reasonable decisions regarding Care and Veterinary

Services, and consents to C aregiver’s rea sonable decisions, acts and omissions, including the risks thereof.

Owner authorizes the euthanasia of Pet if it becomes gravely injured or ill, and a veterinarian

recomm ends either euthanasia or treatment costing in excess o f $ ____________________ (if left blank, then

the am oun t shall be $1,000 .00).
