Senior Civil Servant, Grade Level 10 and above (Please state your grade level on this form, if



applicable).

Business Man/Woman (with an identified registered company’s number with the Corporate



Affairs Commission which must be indicated on the form).

Practicing professional (e.g .Architects, Accountants, Engineers and Lawyers e.t.c) who must



have attained Managerial position or its equivalent.

Traditional Rulers or a Reputable Clergyman.



Senior Banking Officers in Banks or from Deputy Manager of reputable companies.


