3The following document is intended for the use of health care professionals only who
are giving influenza vaccine.
When residents arrive at an Ottawa Public Health (OPH) influenza immunization clinic,
they will be asked to fill out a consent form specific to OPH clinics. Residents should
not bring their own consent form.
