During your child’s life at Rosa Street Primary School, there will be opportunities where

photos/video of your child will be used for curriculum or assessment purposes. These may be

individual/group photos of your child for display purposes or individual/group photos for

assessment records. Also, video or a sound recording may be used for display or assessment

purposes of a child, that cannot be obtained from a still image, e.g. drama. Before taking any

photographs of your child, we need your permission.

Please sign and date the form and return it to school as soon as possible.
