Field Trip Permission Form

Please sign this form and return to school.

During the school year, students are taken on field trips occasionally.

Some of these are in the local area and others are outside the community.

When the students go on trips outside the community, they are

transported by school buses.

We are asking you to sign this permission slip, which will cover all field

trips taken this year by the children in Pre School through eighth grade.

K – 8 Parents will be notified in advance by their child’s teacher of the

specific field trips to be taken.
