1CHARITABLE CONTRIBUTION FORM
It is the corporate policy of PDQ Car Wash to donate car washing and detailing services only in the
form of car wash certificates. Donations are considered to be corporate contributions. For example,
PDQ will not donate car washes from more than one wash location to the same cause but rather make
one corporate donation.
Requests will be honored from all NON-PROFIT organizations that are holding fundraising or other
charitable events. Other requests will be reviewed on an individual basis.
All requests for contributions need to be in writing, either on stationery of the requesting organization
or by filling out this form. The request can be mailed, emailed, faxed or dropped off at the corporate
office. Requests must be made at least two weeks before the date of the event. If two weeks
notice is not provided, PDQ will make every effort to meet the request but cannot guarantee it.
All requests will be reviewed promptly. Upon approval, a car wash certificate will be mailed to the
appropriate contact person. Please print clearly and complete the entire form.
