The purpose of this form is for the Financial Aid Office to ensure that aid corresponds to your enrollment level. Every distance learning student has
an individualized budget for financial aid that must be calculated on an individualized basis each term. This form allows the financial aid counselor
to create your budget before you start classes.

Students can only receive financial aid from ONE institution during any given period of enrollment.

Students must be enrolled in at least one class at Old Dominion University and be enrolled at least halftime (see chart below)
to be eligible to receive federal financial aid. Verify with your Student Success Advisor that the institution has a financial aid
agreement with ODU. If that institution is not a financial aid partner, financial aid will not be awarded for courses taken
there.

To receive your financial aid, the ENROLLMENT FORM must be completed by the student and received by the Old
Dominion University Financial Aid Office.

A student may be eligible for the Federal Pell Grant if enrolled less than half time if all eligibility criteria are met. The
maximum Pell limit is 6 years.

If you are completing coursework with a partner community college you are required to complete the Consortium Agreement
Form
(NOTE: TCC, TNCC, & PDCC do not use this form. Please use the Virginia Tidewater Cross Registration Form.
Your Student Success Advisor can assist you with this process).

If completing a consortium agreement form, financial aid disbursements will be delayed until the ODU Financial Aid Office
receives the consortium agreement form from the community college after its drop/add deadline. NOTE: The consortium
agreement form must be completed and signed by your ODU Student Success Advisor.
