mind taking a few minutes to answer the following questions to help our church determine if you

meet our financial guidelines?

Do you have a valid Georgia Driver’s License or I.D.?

3. Are you a member of another church?

4. Have you applied for financial assistance at this Church in the past 6 months?

5. Have you applied for financial assistance anywhere in the past three months?

6. Do you live within the State of Georgia?

7. Does your financial request exceed $200?

8. Are you facing eviction within the next seven days?

9. Are you applying for assistance with a phone or cable bill?

10. Are you willing to attend financial counseling or a small group Bible Study if asked?
