OPEN ADOPTION RECORDS

A person who was born in the state of Oklahoma and who was adopted after November 1, 1997

may obtain a non-certified copy of his/her original birth record without a court order if all of the

conditions below are met:

1. Adopted person is age 18 or older [10 § 7503-2.5 B and 10 § 7505-6.6 D2] AND

2. Complete an application AND

3. Pay fees – $15 for search and issuance of 1 uncertified copy [10 § 7505-6.6 D and § 310:105-1-3 ] AND

4. Provide valid, legal photo ID [10 § 7505-6.6 1 and § 310:105-1-3 A] AND

5. Provide a signed affidavit (below) [10 § 7505-6.6 1] AND

6. The birth parents did not both sign Affidavits of Nondisclosure. Note: If an Affidavit of Nondisclosure

was filed by only one parent, then the information for that parent must be redacted before the

record can be released. [10 § 7503-2.5 C-D and 10 § 7505-6.6 D2]

In all other cases, a certified copy of the original birth record may be issued if all of the following

conditions are met:

1. Provide a certified copy of a court order [10 § 7505-6.6 E and 63 § 1-316B] AND

2. Complete an application AND

3. Pay fees – $40 for search and issuance of 1 certified copy [§ 310:105-1-3 ] AND

4. Provide valid, legal photo ID [10 § 7505-6.6 1 and § 310:105-1-3 A] AND

5. The birth parents did not both sign Affidavits of Nondisclosure. Note: If an Affidavit of

Nondisclosure was filed by only one parent, then the information for that parent must be redacted

before the record can be released. [10 § 7503-2.5 C-D and 10 § 7505-6.6 D2]
