INSTRUCTIONS

Enter the original financing statement number and date of filing from the original UCC-1F filing. This information is required.

Indicate the intent of the filing by checking the appropriate check box. (continuation, termination, assignment or amendment)

Enter the authorizing secured party of record name. The secured party of record is the most recent secured party submitted to the secretary of
state for filing.
Debtor:
(All debtor entries must include the last four (4) digits of the SSN or TIN.)

Adding a new debtor: Check the ADD box above an unused debtor block, then enter complete new debtor information in the associated fields.
5.
Amending a debtor: Check the AMEND box above an unused debtor block, then enter complete amended debtor information in the associated fields.
6.
Deleting a debtor: Check the DELETE box above an unused debtor block, then enter complete deleted debtor information in the associated fields.
7.
Previously filed debtor information can be included by checking the CURRENT box above an unused debtor block and then entering complete debtor
information from previous filings. Inclusion of current debtor information is purely at the discretion of the filing party, and is not verified by the filing office.
Secured Party:

Add, amend, or delete a secured party: Apply the instructions in numbers 4, 5, and 6 to an unused secured party block, using relevant secured party
information.

For an assignment: Check the ASSIGNEE box above an unused secured party block, then enter the secured party information in the associated fields.

Collateral: To delete a farm product, indicate “D” for delete and enter the collateral information to be deleted. To add a farm product, indicate “A” for add
and enter the collateral information as instructed on form UCC-1F.

Signatures: Debtor signatures and secured party signatures are optional if a signed agreement exists granting a lien on the farm product(s). To use this
form to terminate a UCC1F filing, the secured party signature is required.
10. Payment: Include method of payment - $10.00 if typed, $14.00 if not typed.
Attachments other than form UCC-3Fa are $1.00 per page and are not indexed. Termination of a UCC-1F is free of charge.
