I give permission for myself/my daughter to participate in scheduled events for Southwest California

Pageants. Upon winning one of the titles conferred by Southwest California Pageants, I understand and

agree that I/my daughter will participate in at least two events per month (one event per month for

Children’s titleholders). I further understand that I am responsible for ensuring that I/my daughter has

transportation to and from all events associated with my/her duties as a member of the Court.

Furthermore, in signing below I agree to the Code of Conduct for Southwest California Pageants.

In consideration of my/my daughter’s participation in scheduled events, I hereby take action for myself,

my daughter, my executors, administrators, heirs, next of kin, successors, and assign as follows:

1. Waive, release and discharge from any and all liability Southwest California Pageants; SWC

Pageant Owner, Directors, Coordinators and Volunteers; Event holders, sponsors, directors and

volunteers for Participant’s injury, disability, death, property damage, property theft or actions

of any kind which may hereafter accrue to us, including my and my daughter’s traveling to and

from events.

2. Indemnify and hold harmless SWC Pageants, and its representatives, owner, employees,

coordinators and agents for any damage or injury arising out of my/my daughter’s participation

in the Pageant and pageant-related activities regardless of the cause.

In further consideration of participating in the events scheduled during my/my daughter’s Participation

with Southwest California Pageants, I hereby grant Southwest California Pageants permission to use

participant’s name, statements, information provided in application, photographs and/or likenesses

(including by not limited to photographs, videotapes, motion picture film and/or electronic images as

well as audio recordings made of participant’s voice in whatever form) for promotional, advertising and

media purposes or other lawful purposes authorized by owner of Southwest California Pageants. Any

media bearing my/my daughter’s likeness may be used alone as a member of a group in a composite or

in such a manner as will most favorably serve to promote and advertise Southwest California Pageants

at their discretion. Furthermore, I consent that any such media including photographs, film recordings,

electronic images and/or the tapes and/or the software from which they are made shall be the sole

property of Southwest California Pageants, free and clear of any claim whatsoever on my part in

perpetuity.
