The UPS Store requires you to ship your painting UPS or FEDEX. All paintings MUST
arrive by Monday, September 26th, 2016. All submissions should be properly packed
(see below) and insured. Plexiglass is recommended. Do not ship to the museum.
Include in the box:
1. A PREPAID Return Shipping Label from UPS or FEDEX.
2. A check for handling costs in the amount of $30.00 made out to The UPS Store.
3. A signed copy of this completed form.
4. The 2 part Exhibition Identification Label.
5. Label your box and ALL packing material in your name.

The Connecticut Pastel Society recommends you ship your paintings in a “Strongbox”, available online. Do
NOT ship in a wooden crate and do NOT use loose materials such as peanuts or blankets.
All work will receive careful handling by competent staff, however, neither the Connecticut Pastel Society, The
UPS Store or the Slater Memorial Museum, nor anyone connected with this exhibition accept responsibility for
loss or damage to any work of art submitted. Your signature below constitutes agreement. We recommend that
artists carry their own insurance.
