STUDENT FINANCIAL NEED ASSESSMENT FORM
for Diploma in Operatic Performance Students

Some University of Toronto awards require the recipient to be an Ontario resident who is able to demonstrate
financial need, in addition to other selection criteria. All students who are Ontario residents are requested to
submit this form to ensure than an accurate assessment of financial need can be made.
Ontario residency is defined as follows:
To be eligible, students must be Canadian citizens or permanent residents and satisfy one of the following
criteria:
1) student has always resided in Ontario or
2) Ontario is the last province that the student has resided in for 12 consecutive months without being a
full-time post-secondary student
3) Student who is considered married for OSAP purposes can be considered an Ontario resident if the
student’s partner has resided in Ontario for at least 12 consecutive months immediately before the
last day of the month in which classes began for the student’s most recent period of full-time post-
secondary studies and during this time, the student’s partner was not enrolled in full-time post
secondary studies.
4) a single dependent student can be considered an Ontario resident if the student’s parent(s), step-
parents(s), legal guardian(s), or official sponsor(s) has resided in Ontario for at least 12 consecutive
months immediately before the last day of the month in which classes began for the student’s most
recent period of full-time post secondary studies.

Personal information that you provide to the University is collected pursuant to section 2(14) of the
University of Toronto Act, 1971.
It is collected for the purpose of administering admissions, registration, academic programs, university-
related student activities, activities of student societies, safety, financial assistance and awards, graduation
and university advancement, and reporting to government.
The University is also required to report student-level enrolment-related data to the Ministry of Training,
Colleges and Universities as a condition of its receipt of operating grant funding. The Ministry collects this
enrolment data, which includes limited personal information such as Ontario Education Numbers, student
characteristics and educational outcomes, in order to administer government postsecondary funding, policies
and programs, including planning, evaluation and monitoring activities.
