CONSENT AND MEDIA RELEASE FORM

Please note that all capitalized terms used, but not defined, in this Consent and Media Release Form shall have the

meanings assigned to them in the Official Contest Rules.

*THIS FORM MUST ACCOMPANY EACH CONTEST SUBMISSION*

PARENTS AND/OR LEGAL GUARDIANS please carefully read this Consent and Media Release Form

(“Consent and Release Form”) and the

Official Contest Rules

in their entirety. This Consent and Release Form,

together with the Official Contest Rules, form a binding agreement relating to your minor child’s participation in the

Contest, and/or your agreement, on behalf of your minor child, to be depicted in a work of visual art submitted in the

Contest. Your minor child is not eligible to participate in the Contest until a completed and signed Consent and

Release Form is received by the Sponsor. Contest Submissions not accompanied by the completed and signed

Consent and Release Form will not be considered.

ADULTS (18 YEARS OF AGE OR OLDER) please carefully read this Consent and Release Form and the

Official Contest Rules

in their entirety. This Consent and Release Form, together with the Official Contest Rules,

form a binding agreement relating to your participation in the Contest (as defined in the Official Contest Rules),

and/or your agreement to be depicted in a work of visual art submitted in the Contest. You are not eligible to

participate in the Contest until a completed and signed Consent and Release form is received by the Sponsor.

Contest Submissions not accompanied by the completed and signed Consent and Release Form will not be

considered.

1) I represent and warrant that I have read, understood, and agree to be bound by the Official Contest Rules

located at I acknowledge and

agree that I (or if I am the parent or legal guardian of a minor Student Participant in the Contest, my minor

child) will be bound by these Official Contest Rules and this Consent and Release Form;

2) I represent and warrant that I (or if I am the parent or legal guardian of a minor Student Participant in the

Contest, my minor child) meet(s) all eligibility requirements listed in the Official Contest Rules;

3) I understand and agree that my (or if I am the parent or legal guardian of a minor Student Participant in the

Contest or a minor whose name, image or likeness is depicted in any Submission (“Depicted Person”), my

and/or my minor child’s) full name, age, grade level, city, state, image, and Contest Submission may be

disclosed and displayed publically (off line, in social media sites, press releases, advertising, promotions,

etc.) without any additional notice or financial compensation, or that I and/or my minor child may be

required to participate in any media or promotional events related to the Contest. I further warrant that I

have read, understood and agree to the Sponsor’s

Privacy

Policy;

4) I understand and agree to provide certain rights and releases to the Sponsor as described in the Official

Contest Rules;

5) I hereby warrant that I am a legally competent adult and have the full right and authority to execute this

Consent and Release Form. If I am a parent or legal guardian of a minor child Student Participant in the

Contest or Depicted Person, I further warrant that I am a legal parent or legally appointed guardian of such

a minor child, and that I have the full authority to give consent, to release, and to contract for my minor

child. I also agree to provide any additional information or to execute any additional form(s) that are

necessary for me and/or my minor child to be eligible to participate in the Contest and/or for the school

entering the Contest to claim a prize.
