This 503(b)(9) Claim may be filed in person, by electronic filing, by courier service, by mail or by hand delivery.
All 503(b)(9) Claims must be filed so as to be received by the Bankruptcy Court at the following address: United
States Bankruptcy Court for the District of Minnesota, U.S. Courthouse, Rm. 301, 300 South 4th Street,
Minneapolis, MN 55415. Please note transmission by facsimile or electronic mail will not be accepted.
ANY DOCUMENTS RELIED UPON BY ANY 503(b)(9) CLAIMANT TO SUPPORT ITS 503(b)(9) CLAIM,
INCLUDING ALL DOCUMENTS THAT PURPORT TO ESTABLISH (A) THAT SUCH CLAIMANT
SUPPLIED “GOODS” TO THE DEBTOR THAT WERE RECEIVED WITHIN THE 20-DAY PERIOD
BEFORE MAY 5, 2015, (B) THE “VALUE” OF SUCH GOODS AND (C) THAT SUCH GOODS WERE
“SOLD TO THE DEBTOR IN THE ORDINARY COURSE OF THE DEBTOR’S BUSINESS,” MUST BE
ANNEXED TO THIS 503(b)(9) REQUEST FOR PAYMENT.
To be considered timely filed, all 503(b)(9) Claims must be actually received by the United States Bankruptcy
Court for the District of Minnesota, U.S. Courthouse, Rm. 301, 300 South 4th Street, Minneapolis, MN 55415
by 5:00 p.m. (Central Time) on or before September 1, 2015. All 503(b)(9) Claims must include appropriate
documents/materials establishing the claimant’s entitlement to an allowed claim and the amount of the
asserted claim.
