BOND APPLICATION GUIDELINES

4) Importer No.- Internal Revenue Service number or Social Security , and Customs assigned numbers,
for IRS number please use the following format 12-345678900, the last two digits should always be 00
unless you are adding a division to an existing bond. Social Security number, use the following format
123-45-6789 and make sure that the importer name is the individual’s first and last name. If they have a
DBA name, please include it in the appropriate field. Customs assigned numbers should be in the
following format 071001-00001, the first two digits are the year it was issued, and the next four digits is
the port that issued the number, and the last five digits are random numbers issued by CBP.
5) Address – please make sure the number and address are being reported as the correct mailing
address, CBP uses the source
, they reject any bond application with incorrect
information.
6) Description Merchandise – please describe the merchandise being entered with the same description
as imported on the Foreign Commercial Invoice with the country of origin for each item. Merchandise
subject to anti-dumping requires collateral in order for us to process your application. Acceptable forms
of collateral are Letters of credit issued by a U. S. Bank who is a member of FDIC, or confirmed by U. S.
banks member of the FDIC, and must be rated with 3 stars or better on

7) Last Calendar year/Estimated next Calendar year – list the merchandise, value and number of entries
made in the last calendar year, you may estimate the next Calendar year totals.
8) Type Bond requested – 19CFR 113.62 CBP requires a minimum of a $50,000. Bond for Continuous
bond(s) and the amount of the bond is dependent upon the number of entry(s) from the previous year.
Bonds that are $100,000 and greater require financial statements for underwriting. The statements
should be current, (no more than one year old) and must include a profit and loss statement, and a
complete balance sheet.

10) Other Districts – please list the other districts in the U. S. in which you Import.
11) Current Bond(s) – list any and all current bonds on file with CBP.
12) Certification – By- print name of individual signing the bond application. List the title of signator, and
date. Signature must be legible. By signing the bond you confirm and attest, to the best of your
knowledge, that there are no other known importers or principals associated have caused surety claims.
