Guidance for end-users to complete End User Statement form

This End User Statement (EUS) form must be provided when you order military goods which are to be used by you (whether via a

consignee or not). The documents must be completed on the paper with the logo your company except in the case when you are

a natural person. The End-User Statement will be valid only if the document shall be presented to the Romanian competent

authority in 6 months from the date of signing of the fourth Section

The Romanian Licensee is the person (according to Romanian law) who is either the supplier of

the goods outside Romania, or the broker who moves those goods, each of which may be direct to

the end user or via a separate consignee

If not known, the transfer license references can be left blank for completion by Licensee.

The consignee is the person who is the first recipient of the goods. This can be yourself or it can be

another person within your country or in another country. If you are both the consignee and the end

user, then you must enter your own details in boxes c. and d.

As the end-user you will be receiving the goods for end-use and so such you must fill in boxes e.

and f. If you incorporate the goods into another products or higher level system, then you are still

the end-user for the purposes of the movement of the items.

The end-user in this section refers to the entity at the time of shipment

The goods in this section define those at the time of shipment. If goods are being incorporated into

another product or higher level system then this information should be included in Section 3

The Romanian authorities need to understand what the goods are so a detailed description of the

main item or items is required. If, for example, the main item or items are accompanied by a long

list of spares, components, or accessories, you should also indicate this in page 3 (Annex). For

consumable goods, include length of time supplies are expected to last

Set out the specific purposes for which the goods are to be used by the end user. If the goods are

to be incorporated by you (for example installed into another product or higher level system) then

that product/higher level system, its use and ultimate end user (which may be within or outside your

destination) should be described in Section 3, where known

Section 4 must be signed and dated by you as the end-user of the goods mentioned in Section 2,

in original and authenticated to the Notary Public. This includes the place where you incorporate

the goods into another product or higher level system. This part must be signed by a person within

your company with legal powers to sign on behalf of the company/organization. Digital signatures

are not accepted.

All the parts of the document and the commitments of the end-user must be completed legibly and

in English (or accompanied by a certified translation, including original header details if written in a

foreign language). Correctness of the completion and the English translation must be verified by

the Romanian licensee who has signed the transfer license application behalf of the company.

If the end-user chooses not to use the relevant template, then the Romanian authorities require an

original signed and dated statement in English on their headed paper providing the same

information and assurances

The same responsible official of the end-user should also complete, sign, date and attach a

covering letter. This letter must be on their original official headed paper
