Change of name
notification
This form can be used to notify the Teacher Registration Board of Western Australia
(TRBWA) of a change of name.
All registered teachers must keep their contact details up-to-date with the TRBWA. Regulation 27A of
the Teacher Registration (General) Regulations 2012 (Regulations) requires registered teachers to give
written notice to the TRBWA within 30 days of a change of name, postal address or email address.
If the name you currently use is different to that shown on the Register of Teachers you must provide
evidence of your name change, e.g. Marriage or Change of Name Certificate issued by a State or
Territory Registry of Births, Deaths and Marriages and Divorce papers issued by the Family Court.
Please provide certified copies of supporting evidence (not originals). For further information on
