INSTRUCTIONS

for

CITIZEN DECLARATION FORM

HUD requires that all applicants complete and return a CITIZEN DECLARATION

in order to be considered for tenancy at Trinity Tower. You are required to declare

your U.S. Citizenship, or submit evidence of eligible immigration status.

Please read all instructions (including those on the form) before supplying the

requested information. Fill in all lines that are applicable.

A separate completed CITIZEN DECLARATION is required for each household

member who will reside at Trinity Tower.

Your application will be returned if received without a completed CITIZEN

DECLARATION.

Please note the following definitions:

Alien Registration No. – A seven or eight-digit number assigned to an

individual at the time their immigration file is created. Persons born in the

United States will not have an Alien Registration Number.

SAVE Verification No. – An automated or manual information-sharing

program whereby state agencies may verify the immigration status of alien

applicants for entitlement benefits. PLEASE LEAVE THIS LINE BLANK

Nationality – Is the country you are a citizen of, and owe allegiance to (ie:

USA). A common mistake people make is to list their ethnicity. For the

purposes of the CITIZEN DECLARATION, please only list the country you

are a citizen of.
