Please complete, sign and date this form to enable us to ascertain whether you qualify for treatment as an accredited investor

I/We declare and represent in favour of National Australia Bank (“NAB”) that I/we am/are an accredited investor as defined in Section 4A()(a)

of the Securities and Futures Act (Chapter 289 of Singapore) (the “SFA”), namely (please tick all boxes that apply):

individual(s) whose net personal assets exceed in value S$2 million (or its equivalent in a foreign currency);

individual(s) whose income in the preceding 2 months is not less than S$300,000 (or its equivalent in a foreign currency);

a corporation with net assets exceeding S$0 million in value (or its equivalent in a foreign currency), as determined by:

a. the most recent audited balance-sheet of the corporation; or

b. where the corporation is not required to prepare audited accounts regularly, a balance-sheet of the corporation certified by the

corporation as giving a true and fair view of the state of affairs of the corporation as of the date of the balance-sheet, which date shall

be within the preceding 2 months;

the trustee of a trust of which all property and rights of any kind whatsoever held on trust for the beneficiaries of the trust exceed S$0

million in value (or its equivalent in a foreign currency);

an entity (other than a corporation) with net assets exceeding S$0 million in value (or its equivalent in a foreign currency);

a partnership (other than a limited liability partnership within the meaning of the Limited Liability Partnerships Act, Chapter 63A of

Singapore) in which each partner is an accredited investor; or

a corporation, the sole business of which is to hold investments and the entire share capital of which is owned by one or more persons,

each of whom is an accredited investor;

I/We undertake to promptly inform NAB if I/we cease at any time to be an “accredited investor”.

I/We have attached the following supporting documentation to substantiate my/our declaration above that I/we qualify for treatment as an

accredited investor:

latest income tax assessment for the year of assessment _________________________________________ ;

an audited balance-sheet for the financial year _________________________________________ ;

a balance-sheet dated _________________________________________ certified by us as giving a true and fair view of our state of affairs; or

I/We understand that the above declaration is subject to NAB’s internal assessment of my/our status and that NAB shall retain sole and absolute

discretion as to my/our classification as an accredited investor. NAB shall not be required to provide any reason or explanation as to the

classification of my/our status.

I/We understand, acknowledge and agree that should I/we be classified as an accredited investor, NAB will be exempt from complying with

certain regulatory compliance requirements under the SFA and the Financial Advisers Act (Chapter 0 of Singapore) as set out in the [Annex] to

this declaration.

I/We confirm that all my/our particulars in this declaration are true and correct to the best of my/our knowledge and belief.
