DIRECTIONS FOR COMPLETION OF INDIVIDUALIZED STUDY FORM
All portions of the form above “Required Signatures” must be completed by the student with the
exception of sections marked “Justification for the Independent Study” and “Method of Evaluation” and
“Title of Study” which should be completed by the instructor in consultation with the student. If more
space is needed, an additional sheet may be attached to the form. The form should be typewritten or
written neatly in ink.
Students should have a 3.00 cumulative GPA before attempting independent study
course work. The student’s cumulative grade average as of the last completed term must be reported on
the form. Normally, students may register for only one independent/directed study in any single term. In
addition, regularly offered courses normally may not be taken as independent study. Exceptions to
independent study policies may only be approved by the Associate Dean.
There are three types of individualized studies.
DIRECTED STUDY (400): An individualized course in a specific topic in which the student
meets regularly with a faculty member to receive instruction.
INDEPENDENT STUDY (401): An individualized course or project in which the student
proceeds independently on his or her study, and meets with a faculty member for a limited number of
times to report progress and to receive suggestions.
RESEARCH PARTICIPATION (402): An individualized course or project in which the student
conducts research in an area which is a part of a faculty member’s own area of research interest and
competence.
Usually, Individualized Studies are done during one term and are given three hours of credit.
However, students may have their study extended over two or three terms and, in some cases, students
may receive more or less than three hours credit.
Students are registered for independent studies by the Registrar’s Office as soon as the completed
form is submitted to the office with the required signatures.
It is the student’s responsibility to secure the approval of the Instructor, Program Chair, and
Associate Dean.
DEADLINES: Completed forms (including all required signatures) must be delivered in person by the
student to the Registrar’s Office. Normally, independent study forms should be submitted during the
registration period for the term of the study (in April for fall term independent studies and in November
for CentreTerm and spring term independent studies). However, the Associate Dean will accept forms for
review through the first week of classes for fall and spring terms and through the first day of classes for
CentreTerm.
