SUBLEASE AGREEMENT

The following sublease agreement must be submitted and approved before you can move forward

with your sublet.

**Please note there is a fifty dollar ($50.00) sublease fee that will be assess to the lessor’s account

upon acceptance of this application.

It is the responsibility of the sublessor to obtain a copy of the lease from the lessor so you aware of

your rights and responsibilities.

The security deposit of the original lessor shall be retained by DHB until the end of the original

contract date. If the lessor chooses he/she can collect a deposit from the sublessor and make lease

end arrangements for this deposit. This agreement shall be between the lessor and sublessor, not

DHB.

The sublessor must submit a copy of his/her photo id with this agreement.

The lessor must continue to pay the rent each month to DHB. The sublessor must pay the agreed

upon amount directly to the lessor. The only options for the sublessor to pay rent directly to DHB are

(1) mail a check or money order to DHB, 753 Genesee St, Rochester, NY 14611. The check must

include the property address and the name of the lessor on the memo line or (2) the lessor can

provide the sublessor with your online account login information. The sublessor will not have their

own account. The sublessor will be a subcontact on the lessors account.

As the lessor, it will be your responsibility to relay any messages from DHB to the sublessor regarding

property visits, lease end information etc.

We will also need a written approval from each resident of the property acknowledging their

acceptance of this sublet.
