(Active Legion of Honor, Honorary Legion of Honor, Cross of Honor and Chevalier)
In accordance with the DeMolay Rules and Regulations, Article 6, Section 206.4(a): “Nominations shall be made
without the knowledge of the nominee. Any person who fails to observe secrecy as to consideration for or action on any
nomination for any honor shall himself be subject to removal of his own honors and to removal from his office and
memberships.”
Nomination Fee: The nomination fees as established by DeMolay International must be attached to this form when it is
submitted. Make checks payable to DeMolay International.

Active Legion of Honor - $75.00
Honorary Legion of Honor - $75.00
Cross of Honor - $50.00
Chevalier - $45.00
Active Legion of Honor: Demolay International may confer the DeMolay Legion of Honor upon a Senior DeMolay,
who has attained the age of twenty-five years as of the first day of the annual meeting of the year nominated, for
outstanding leadership in some field of endeavor, or for success in fraternal life including adult service to the Order.
Honorary Legion of Honor: DeMolay International may confer the DeMolay Legion of Honor upon a Mason who is
not Senior DeMolay, who has attained the age of thirty years as of the first day of the annual meeting of the year
nominated, and who has performed unusual and meritorious service in behalf of the Order or who has evidenced a spirit
of cooperation and appreciation for the Order. It shall not be given only for service on an Advisory Council.
Cross of Honor: DeMolay International may confer the DeMolay Cross of Honor upon a present or past member of an
Advisory Council or upon any personal representative of an Executive Officer who has served at least three years in
either or both capacities and whose service has been conspicuously meritorious.
Chevalier: DeMolay International may confer the Degree of Chevalier upon a member of the Order or a Senior
DeMolay who has performed unusual and meritorious service in behalf of the Order, who has attained the age of
seventeen years as of the first day of the annual meeting of the year nominated and has been a member in good standing
for at least two years on that date. An Executive Officer may lower the age eligibility requirements to sixteen.
Nominations: Nominations for any of the four above named honors must be made to DeMolay International by an
Executive Officer. An Advisory Council of a chapter may make recommendations for any of these honors to their
Executive Officer, who may nominate, if he desires, such nominee, as he deems worthy.
Nomination Form: Please complete this form so that all information is clear and legible.
Regalia: In the case of LOH, HLOH, and Chevalier, the ritual requires the presentation of the appropriate Cordon,
Medallion and Patent of Membership. The COH requires the presentation of the Breast Jewel and Patent Membership.
The regalia in all cases will be sent to the Executive Officer of a Jurisdiction, or his appointed representative.
Ring Optional: In the case of the LOH, HLOH and Chevalier, a ring is available on an optional basis, and information
on the ring will be included when the nominee is notified of his election.
Investiture: The investiture for the LOH, HOLH and conferral of the COH is under the direction of the Executive
Officer for each Jurisdiction. The Executive Officer will notify the Honoree-elect when and where the investiture will be
made. The investiture for Chevalier is under the direction of the Executive Officer, or each Chapter Advisory Council,
as the Executive Officer may direct.
Deadlines: The nomination form must reach the Executive Officer in your Jurisdiction by the Jurisdiction established
deadline. This nomination form must reach the Grand Secretary no later than April 1 for the year nominated.
