2School/ Center Retainer Agreement
I, ___________________________________________________________, authorized representative
of _______________________________________________________________________enter into an
agreement to retain The Ollena Center for Early Childhood Assessment, LLC for the period of
_______________ school year(s) (August 15th thru May 15th) for the sum of $12,000.00 (Twelve
thousand dollars) per school year to provide the following services:
1. Up to 10 one hour classroom observations
2. Gesell Developmental Observation Record assessments performed for up to 8 students
3. Parent/Teacher conference assessments for up to 100 students in the fall and up to 100 student the
spring, students ages 2-5
4. Initial consultation with School Director, or representative, to understand the schools’/centers’
needs and culture
5. Mention The Ollena Center as “in partnership” with your school in your printed materials and on
your school website
6. Participation at 2 events during the school year, such as, Open House, Parent’s Night, New Parent
Orientation (dates subject to availability)
7. Availability to school faculty as a resource for questions and concerns
8. Coordination of facilitating outside therapists visiting the school to conduct paid screenings, such
as, speech and language, vision, or occupational therapy (third party screening fees may apply)
9. Coordination of special services through the appropriate county: Babies Can’t Wait, Fulton/Cobb
etc County Schools, Services for Exceptional Children Preschool, including assisting with filing
paperwork, speaking with parents
Up to 5 individual observation and assessments for up to 5 specific students
10.
At that time:
Parent signs permission form for observation/assessment
Classroom observation of their child
Written report of observation findings given to parents
Meeting with child’s parents and teacher at school to discuss findings and develop a plan going
forward
