3. All copies of required enclosures to the PAN Application.

I authorize you to debit Rs 56(Rs 50 plus service charges of Rs 6) to my/our

account towards your bank charges.

I also authorize you to forward the above said documents viz. application for

PAN Card, its enclosures, cheque/DD/amount submitted by me to M/s Unit

Trust of India at my risk and responsibility. I understand that PAN Card would
