the business trust named below and for that purpose submits the following statements:

3. The date the business trust registered to do business in Kentucky is ______________________________.
4. The business trust is not transacting business in Kentucky and hereby cancels its registration.
5. The cancellation of registration by the foreign business trust will not terminate the authority of the Secretary
of State to accept service of process on the foreign business trust with respect to causes of action arising
out of the transaction of business in Kentucky.
