Local Services Tax Individual Return
The Local Services Tax (generally referred to as “LST”) is due from certain individuals
based on the taxing jurisdiction of the employer. Federal employees whose LST is not
withheld from payroll and individuals who earn income through self employment or operation
of a business or profession must also report and pay the Local Services Tax. Your specific
tax rate can be determined by visiting the Pennsylvania Department of Community &
Economic Development’s (DCED) website at If your tax rate is greater
than $10.00, you have the option of remitting the payment in quarterly installments using the
LST-I quarterly voucher form found on page 2 or making a one time payment using the
Annual Payment Voucher below. A Tax rate of $10.00 or less must be paid as a one time
annual payment only.

If you previously paid the 2015 tax to another taxing jurisdiction, please provide proof of
payment for credit or adjustment of tax due. If you have questions regarding the tax, please
