REGISTRATION INFORMATION

The Carol Ships Parade of Lights Society is a non profit society dedicated to the promotion of tourism in the Greater

Vancouver area in conjunction with the development of interrelated marine activities. A portion of funds generated by the

Society are reserved for the distribution to charitable organizations.

Confirmation of Society Membership, a pending Carolship Schedule and other related information will be forwarded pending

receipt, at the above noted address, of this completed and signed Registration form accompanied by applicable Membership

Dues.

theCANADA SHIPPING ACT and/or TRANSPORT CANADA in respect to a vessel of this size and type.

I declare that the above named vessel will at all times during the Carol Ship Cruises be operated in a safe and responsible

manner in accordance with all international and local marine rules and regulations.

I acknowledge and agree that the vessel Owner and/or Operator is solely responsiblefor the safety of the vessel and/or

persons or property on board, and Hold Harmless the Carol ships Parade of Lights Society, its Directors, Agents,

Employees or Contractors of the Society, in the event of an injury and/or incident.

Carol Ships Parade of Lights Socity Membership is conditional in the acceptance of terms and conditions as documented
