Schedule CD Instructions
General Information
• You must complete a separate schedule for each state into which you shipped or sold cigarettes.
• Do not include any cigarettes with Illinois tax stamps affixed that you
• returned to manufacturers;
and/or
• sold to Illinois licensed out-of-state distributors.
• Attach two copies of this schedule to either Form RC-6, Cigarette Revenue Return, or Form RC-6-A, Out-of-State Cigarette Revenue Return.
Column 1:
Date you shipped cigarettes out of state, include the month, day and year.
Column 2:
How the cigarettes were shipped, if by
• distributor truck write DT
• common carrier write CC
• parcel post write PP
• customer truck write CT.
Column 3:
Invoice number for the cigarettes you shipped into another state.
Column 4:
This is the Federal Employers’ Identification number (FEIN) of the person or company listed in Column 5.
Column 5:
Name, address (including the city)of the company or person to whom you shipped or sold cigarettes.
Column 6:
Number of packages containing 20 cigarettes that you shipped or sold.
Column 7:
Number of packages containing 25 cigarettes that you shipped or sold.
Column 8:
Number of packages containing cigarettes in quantities other than 20 or 25 that you shipped or sold.
Column 9:
Total number of cigarette sticks per invoice.
Column 10:
Indicate whether or not tax was paid to the state into which you shipped or transferred the cigarettes.
