CERTIFICATE OF DATE OF BIRTH
Requirements:
1. Current/expired Indian passport with valid immigration papers in original
and photocopy. Fees: C$26.00 + $2 Consular surcharge.
MISCELLANEOUS SERVICE ON A PASSPORT
(Change in address, inclusion of spouse’s name, date/place of birth, etc)
This service is provided at BLS International
Requirements:
1. Current passport with copy of the relevant pages.
2. Original landed paper or immigration document with a photocopy.
3. Document in support of the change required. (In case of change in
date/place of birth, an Order of a Civil Judge from India is required).
4. Fees: C$26.00 + $2 Consular surcharge
DEATH CASES
Registration:
Death of all Indian nationals should be registered with the Consulate by
submitting the prescribed application in duplicate together with the death
certificate, burial/cremation certificate, passport of the deceased and fee
of C$1.00 payable to Consulate General of India, Toronto by Bank
draft/Money Order/Certified Cheque.
Requirements for transportation of dead body to India:
1.
Registration of Death with the Consulate (in case of Indian nationals
only).
2.
Death Certificate in original and a photocopy .
3.
Burial permit in original and a photocopy .
4.
Letter from Public Health Office indicating “No Communicable disease”.
5.
Embalming Certificate, Coroners Out of Province Certificate
6.
Flight details and , Name, address, telephone number, etc of the
consignee.
Requirements for carrying of ashes:
1.
Registration of Death with the Consulate (in case of Indian nationals
only).
2.
Death Certificate in original and photocopy.
3.
Cremation certificate in original and photocopy.
4.
Certificate of packing indicating name, age, date, time and cause of
death, contents of package.
5.
Passport of the deceased in original and photocopy.
6.
Name, address and passport particulars of the person taking the ashes
and a copy of the passport.
Fees: C$ 48/- (for Canadian nationals of Indian origin) payable to Consulate
General of India, Toronto by Bank draft/Money Order/Certified Cheque.
