PROVISIONS OF LAW:
Section 320.0846, Florida Statutes, provides that any owner or lessee of a motor vehicle who resides in this state
and is an active member of the Florida National Guard may, upon application and proof of eligibility, be issued one
standard license plate without charge. Applications for any additional license plates must be accompanied by
appropriate fees. Eligible applicants of the Florida National Guard may apply for a specialty license plate as
provided in s. 320.08056 upon payment of the fees required in that section. All other fees will be waived.
Applications for any additional specialty/personalized license plates must be accompanied by all appropriate fees
established in this chapter. The Initial Registration fee ($225) is not waived, if applicable.
Section 320.089, Florida Statutes, provides that the license plates specified on the reverse side of this form may
only be issued to the following types of vehicles:
Automobiles (private or lease).
Trucks for private use (weighing 7,999 pounds or less).
Motor homes or truck campers which are NOT "for hire" or "commercial" use.
INSTRUCTIONS:
A.
Sections 1 and 2 on the reverse side of this form should be completed if requesting a US Reserve or a
Florida National Guard License Plate.
The following information will help you to choose the correct box for the Florida National Guard License
Plate in section 2 on the reverse side of this form:
NOTE: An active Florida National Guard may be issued ONE FREE license plate/registration
each year, however, any specialty and personalized fees would still apply, if
applicable.
Box A:
When an active Florida National Guard member is applying for a free original Florida
National Guard license plate and registration.
Box B:
When an active Florida National Guard member is applying for a free original
regular/specialty/personalized (not National Guard plate) license plate and registration in
lieu of a National Guard license plate. If requesting a specialty or personalized plate,
specialty or personalized fees would apply, if applicable.
Box C:
When an active Florida National Guard member is requesting their annual renewal of their
Florida National Guard, regular, specialty or personalized (specialty or personalized fees
would apply, if applicable) license plate.
Box D:
When a retired or active Florida National Guard member is applying for an original National
Guard license plate that is not exempt from license plate and registration fees. The retired
member must also submit proof of their retirement, such as DD214, etc.
Box E:
When an active Florida National Guard member is renewing their Florida National Guard
license plate that is not exempt from registration fees (i.e., already received the free annual
renewal for another license plate, additional plates, etc.). A retired National Guard member
is not required to submit this form for renewal of their Florida National Guard license plate;
however, the member must submit proof of their retirement, such as DD214, etc.
NOTE: Multiple boxes may be checked, if applicable.
B.
Sections 1 and 3 on the reverse side of this form should be completed if requesting an Ex-Prisoner of
War, Purple Heart, Pearl Harbor Survivor, US Paratrooper, Operation Iraqi Freedom, Operation
Enduring Freedom, Gold Star, or Fraternal Order of Police License Plate.
