IMPORTANT: READ ALL INSTRUCTIONS CAREFULLY BEFORE COMPLETING FORM.

FILING DEADLINE: Pursuant to NRS 78.150 Initial List of Officers, Directors

and Resident Agent form MUST be filed on or before the first day of the

second month following filing date. Example: If corporation filed on

October 15, 1999 list must be filed by December 1, 1999.

TYPE or PRINT the following information on the Initial List of Officers, Directors and Resident Agent Form:

1. The FILE NUMBER of the corporation.

2. The NAME of the corporation EXACTLY as it appears in the Articles of Incorporation.

3. The DATE of incorporation.

4. The STATE, TERRITORY or COUNTRY where the corporation is organized should be entered on the

third line after the letter A and before the word CORPORATION.

5. The FILING PERIOD is the month and year of incorporation TO the month and year 12 months from

that date. Example: if the incorporation date was 1/12/99 the filing period would be 1/1999 to 1/2000.

6. The names and addresses of the RESIDENT AGENT, PRESIDENT, SECRETARY, TREASURER and

DIRECTORS should be entered in the boxes provided on the form.

7. The SIGNATURE of an OFFICER, including his/her title and date signed MUST be included in the areas

provided at the bottom of the form.

8. Form and applicable FEES and PENALTIES must be returned to the Secretary of State.

FILE STAMPED COPIES: If you wish to have the page 2 certificate file stamped and returned, enclose a

self-

addressed stamped envelope.

CERTIFIED COPIES: You must send in the number of copies you would like certified and returned to you in addition

to the original list to be filed. A filing fee of $10.00 for each certification is required. Copies received without the

required fee will be returned uncertified. The Secretary of State keeps the original filing.

EXPEDITE FEE: Filing may be expedited for an additional $25.00 fee.

NOTE:

The Las Vegas Satellite Office will only accept

expedited

filings. Regular filings must be sent to the Carson

City address.
