For I ssuance o f F orm I -‐20 o r D S-‐2019

In order to receive a Form I-20 or DS-2019, prospective F-1 and J-1 students must provide documents that

demonstrate adequate finances to fund educational and living expenses for at least one academic year.

Having obtained an I-20, signing it, and either entering the U.S. or filing a change of status to F-1, you agree to

have funds available for the duration of your studies. If financial documentation will come from an individual

sponsor and not your government, organization, or your own personal finances, please also have your sponsor

complete the form below. Each individual contributing financially to your education must complete this form

and submit an official dated bank statement or bank letter on original bank letterhead, which is no more than

six months old and which includes account number(s), account type(s), the current and average balance with a

clear indication of the currency, in English language or accompanied by an English translation, and which is

signed and dated by an appropriate official of the bank or other financial institution.
