Doggy Day Care Liability & Agreement Form

1. I, the owner, acknowledge that my pet is required to have proof of up-to-date

vaccinations which include rabies, Distemper, and Bordetella (kennel cough) in order to

participate in doggy day care.

2. I certify that my dog is in good health and has not been ill with any noticeable conditions

or diseases in the past 30 days.

3. I acknowledge that Wil-A-Lo Kennels, it’s staff and owners, will not be liable for any

injury or financial responsibility from an injury caused to my pet while at doggie day care.

I will be held fully and financially responsible if any injury should occur.

4. I further acknowledge that Wil-A-Lo Kennels, it’s staff and owners, will not be liable for

any injury or financial responsibility from an injury caused by my pet to another pet while

at doggy daycare. I will be held fully and financially responsible, for my pet and the other

owner’s, if any injury should occur.

5. My dog’s attendance in an interactive social environment at doggy daycare is not without

risk. Every effort will be made by staff members to provide a safe environment for my

dog. If an emergency should occur, I will be contacted by a staff member immediately.

Vet care will be provided by Westfield Veterinary Hospital at my expense.

6. I acknowledge that my dog is not a “vicious” breed listed in the City of Des Moines

ordinance Article II, Section 18-41. I further understand that if my dog’s breed is

Staffordshire Terrier, American Staffordshire, American Pit Bull Terrier, or a breed in

which appearance or characteristics are predominantly of those, my dog cannot

participate in doggy daycare because of the City of Des Moines ordinance, not by Wil-A-

Lo Kennel’s choice to exclude breeds from participating in doggy daycare.

7. I understand that Wil-A-Lo Kennels has the right to dismiss my dog from daycare if my

dog expresses any aggressive or disruptive behavior to staff members or any other dog.

The problem will be addressed to me upon pick-up. I further acknowledge that Wil-A-Lo

Kennels serves the right to terminate a doggy daycare agreement if the aggressive

behavior continually occurs after the first offense.

8. I acknowledge that I was truthful and honest during the enrollment interview to the best

of my knowledge about my dog’s behavior and health.

9. I understand that the first day of doggy daycare is considered as a “behavior evaluation”

of my pet to officially be enrolled in doggy daycare. A report card and a thorough

pass/fail evaluation will be given to me upon pick-up that day. I further understand that I

will be charged the equal amount of one day of daycare even if my dog does not pass

the behavior evaluation.

10. I consent to allow pictures taken of my dog while at doggy daycare and posted on social

media or advertising if needed for the promotion of Wil-A-Lo Kennels Doggy Daycare.

11. I have read and understand all rules and regulations. I agree and will abide to all rules

and regulations, terms, and conditions in this agreement.
