DO NOT COMPLETE THIS FORM UNLESS YOU WISH YOUR CHILD TO PARTICIPATE IN THESE ACTIVITIES.
PARENTAL FIREARM AUTHORIZATION FORM
CONSENT OF MINOR TO USE FIREARMS, AMMUNITION, AND B.B. DEVICES*
I, the undersigned custodial parent or legal guardian of
__________________________________________, a minor, do hereby authorize the Silicon Valley Monterey
Bay Council to furnish firearms,
muzzle loading rifle,
ammunition, and B.B. devices, as appropriate*, to the
minor named herein for the purpose of instruction in the safe handling and shooting of firearms, target shooting,
and related activities under the supervision of the shooting sports director, range master, or range staff.
I do further agree to indemnify and hold harmless the Boy Scouts of America, the Silicon Valley Monterey Bay
Council, its chartered organizations, and all officers, members, employees, and volunteers thereof, from all
suits or actions brought for, or on account of, any injuries or damages received or sustained by any person or
persons by or from the consequences of any negligence or any act or omission of the above named minor
occurring during the course of said instruction.
This authorization will remain in effect for said minor while participating in any Boy Scouts of America program
or related activity related to firearms, ammunition, and B.B. devices unless revoked in writing by the
undersigned and said revocation is personally delivered to the Silicon Valley Monterey Bay Council.

* The State of California has enacted legislation that requires the consent of a minor’s parent or guardian before a firearm, live ammunition
or B.B. devices may be furnished to the minor for the purpose of instruction in the safe handling and shooting of guns and related activities.
Adult Participation
All Day Camps and Twilight Camps: An adult must attend at least two days of camp OR a pack must provide a ratio
of 1:4, adult to youth, each day.
Webelos Resident Camp: A minimum ratio of 1:4, adult to youth, is required.

Health Form and Permission Form for Cubs and Adults
All camps : The BSA health form
(parts A and
B), a copy of medical card and the permission to use a firearm (for
the BB guns) are to be completed and turned in for the registration to be considered complete. These forms are requested
at the time of registration.

these forms should be submitted with the
camp registration form at the time of registration.
The health form is required for both youth AND adults. To comply
with confidential information standards, the health form should not be emailed to the service center.
Cancellation Policy: If there are any cancellations, please notify the Activities Director in writing at least 14 days
prior to the committed date. All refunds/transfers are subject to a 25% cancellation fee. There are no refunds 14 days before
the event. The event will be held rain or shine. No refunds due to weather.
