Important Warning:
Misusing this report will result in severe penalties ranging from a letter of reprimand to a one (1) year suspension from
all MYSA activities for the people involved. This card was developed for the convenience of all involved – do not
abuse it.
Instructions:
This is an official document of the Minnesota Youth Soccer Association. You are responsible for seeing that the
referee fills out the back of this card if one of your participants is sent off. If needed, this card may be used as a
temporary card while the participant pass makes its way back to you. For the match(s) when the participant is serving
his/her suspension, please have the referee sign the back of this card indicating that the participants did not participate
in the match.
To use this card as a participant pass:
1. Inform the referee of this situation and that the participant in question will be participating with this card as
identification.
2. If the referee is not familiar with this procedure, have the referee read the next section of this card titled
“Instructions to Referees.”
3. This card may be used until the original participant pass is returned to you. If it is determined that the participant
pass has been lost, please make arrangements to get another one made.
Instructions to Referees:
This is an official document of the Minnesota Youth Soccer Association. It is intended as a receipt for the pass
confiscated by the referee at the time a participant has been sent off. If you send a participant off, you are required to
fill out the back part completely and return it to the coach of the offending participant, or a responsible adult if you
have sent off the coach.
A coach may present this card to you and ask that you sign it and date it indicating that the participant in question is not
participating in the match, and therefore serving one (1) match of the suspension. A registered adult may also present
this card to you and ask that you sign and date it indicating that the participant (coach) is not participating in the match,
and therefore is serving one (1) match of the suspension.
A coach or registered adult may present this card to you and request that it be used as proper MYSA identification. It is
your obligation as a referee to accept this card and to note on your match report the name of the participant and the
team who presented the card to you. The participant need not be present at the match.
Caution (yellow card) Codes

is guilty of unsporting behavior
C-2
shows dissent by word or action
C-3
persistently infringes the Laws of the Match
C-4
delays the restart of play
C-5
fails to respect the required distance when play is restarted with a corner kick or free kick
C-6
enters or re-enters the field of play without the referee’s permission
C-7
deliberately leaves the field of play without the referee’s permission
Ejection (red card) Codes
E-1
is guilty of serious foul play
E-2
is guilty of violent conduct
E-3
spits at an opponent or any other person
E-4*
denies an opponent a goal or an obvious goal-scoring opportunity by deliberately handling the ball (this does
not apply to a goalkeeper within his own penalty area)
E-5
denies an obvious goal-scoring opportunity to an opponent moving towards the participant’s goal by an
offence punishable by a free kick or a penalty kick
E-6
uses offensive, insulting or abusive language
E-7*
receives a second caution in the same match
* Single match suspension
All other “E” type ejections are a minimum two match suspension.
Adult Dismissal Code
D-1
irresponsible behavior (minimum two match suspension)
The ejected participant does not need to be present at the match in order for the pink card to be signed by the
referee.
