The Plaintiff affirms, under the penalties for perjury, that the following representations are true:

1. That Plaintiff recovered a judgment against the Defendant and that said judgment is still unpaid.

2. That Plaintiff has no cause to believe that levy of execution against Defendant will satisfy said judgment.

3. That Plaintiff believes and has good reason to believe that the Third Party Defendant is now and will be from time to time indebted to the Defendant, which

indebtedness is subject to execution.

WHEREFORE, Plaintiff prays that this court issue an Order requiring the above third Party Defendant to answer interrogatories concerning its indebtedness to

Defendant.
