1. Clearly mark as “Confidential” any Schedule or Attachment included in this
report for which confidential treatment is requested.
2. Class A telecommunications utilities are defined as those having $100,000,000
or more annual revenues from regulated telecommunications operations.
Class B telecommunications utilities are defined as those having less than
$100,000,000 annual revenues from regulated telecommunications operations.
3. This report, properly completed and verified, is to be filed with the Director of the
Public Utility Division of the Oklahoma Corporation Commission, 580 Jim Thorpe
Office Building, P.O. Box 52000-2000, Oklahoma City, OK, 73152-2000, on or
before May 1 of the year following the year for which the report is made. One
copy of this report is to be retained by respondent for reference.
4. It is preferred that all data be typed on this form. If this is not possible, the form
should be completed using black or blue ink. Follow all instructions and answer
each question fully and accurately. The expression “None” or “Not Applicable”
shall be given as the answer there it truly states the facts.
5. The word “Respondent”, wherever used in this report, means the person,
corporation, licensee, agency, authority or other legal entity or instrumentality in
whose behalf the report is made.
6. Financial information in this Annual Report should be reported on an Oklahoma-
specific basis.
7. Access Lines are defined, in context of this report, as all individual business and
residence lines, key trunk lines, PBX trunk lines, and Centrex main lines with
separate numbers.
