REQUIREMENTS FOR ePassport APPLICATION
A.
PASSPORT RENEWAL
1. Personal appearance of the applicant.
2. Latest passport (original) and photocopy of its data page and amendment page, if applicable.
3. Duly accomplished application form.
4. Permesso di Soggiorno and/or Carta D'identità (if applicant is documented).
NSO Birth Certificate and/or other old Philippine issued ID's (if applicant is not documented).
5. Other pertinent documents as may be required.
6. Authenticated NSO Marriage Certificate (original and photocopy) - For a married woman adopting the
surname of her husband for the first time.
7. Authenticated NSO Death Certificate of husband or Authenticated NSO Marriage Certificate with
annotation on the nullity of marriage (original and photocopy).
8. Passport fee of € 54.00.
B.
NEWBORN / FIRST TIME APPLICANT
1. Personal appearance of the applicant.
2. Report of Birth
(separate
application).
3. Other pertinent documents as may be required.
4. Application fee of € 54.00.
C.
REPLACEMENT OF LOST PASSPORT
1. Personal appearance of the applicant.
2. Duly accomplished application form.
3. Authenticated NSO Birth Certificate (original and photocopy).
4. Authenticated NSO Marriage Certificate (original and photocopy) - For a married woman who adopted the
surname of her husband.
5. Affidavit of Loss (sinumpaang salaysay) with fee of € 22.50.
6. Police Report (denuncia).
6. Passport fee of € 135.00 (for lost ePassport) or € 81.00 (for lost MRRP or MRP).
7. Other pertinent documents as may be required, e.g. (Permesso di Soggiorno, Carta D'identità, SSS ID
PRC ID, COMELEC ID, etc.).
Note: a 17 day clearing period is required for application for replacement of lost passport.
D.
APPLICANTS WHO REACQUIRED THEIR FILIPINO CITIZENSHIP UNDER R.A 9225
1. Personal appearance of the applicant.
2. Latest Philippine passport (original and photocopy).
3. Latest Italian passport (original and photocopy).
4. Oath of Allegiance (original and photocopy).
OTHER REQUIREMENTS IF PASSPORT TO BE RENEWED WAS NOT ISSUED BY THE CONSULATE
1. Authenticated NSO Birth Certificate (original and photocopy).
2. Authenticated NSO Marriage Certificate (original and photocopy) - For a married woman who adopted the
surname of her husband.
3. Valid VISA on passport (photocopy).
IMPORTANT REMINDERS
1. The release date of new passport will be indicated in the applicant's official receipt.
2. Passport shall only be released to the applicant or his/her duly authorized representative (with proper
identification and an authorization letter or Delega signed by the applicant). Passport of minors/below 18
years can only be claimed by the parent or guardian who submitted and signed the application. Proof of
parenthood must be presented (i.e. birth certificate of child, permesso di soggiorno of parent showing the
name of the child as dependent/carico).
4. To claim the new passports, please bring the previous passport and official receipt and authorization letter
(for representative claimants).
NO PASSPORT SHALL BE RELEASED WITHOUT THE SAID DOCUMENTS.
5. The Philippine Consulate General only accepts birth/marriage/death certificates issued by the National
Statistics Office (NSO) duly authenticated by the Department of Foreign Affairs (DFA).
