These guidelines are provided to help your organization
prepare a complete grant application.

REPARATION AND
UBMISSION
Your grant proposal must be submitted in English using the most current RMHC Grant Application form
available on our website at the time of application. In addition to the completed grant application form, you
must submit all the following documents in duplicate:
Cover letter on stationery, signed by the senior management official, briefly outlining:
The background of the organization
•
Nature of the proposal and request
•
Concise description of the needs
•
Specific purpose of the funds requested
•
Amount requested
•
Endorsement letter from a McDonald’s employee or licensee or RMHC Trustee (if applicable)
•
Letters of Endorsement are not required for grant consideration
Budget and financial statements must include all of the following:
•
Itemized Project Budget for specific funds requested. This Project Budget must total the amount
of grant monies requested.
Your Operating Budget may not be substituted for your Project
Budget.
Current year’s operating budget
•
Current year’s balance sheet
•
Latest audited financial statements
•
Most current 990
•
IRS 501(c)(3) ruling confirming the organization tax exempt status, and including EIN
Past Donor Information
•
List all private, corporate and foundation support over $500 during the past 12 months
•
Even if this information is embedded in financial documents, please submit a separate sheet this

❖ Follow-Up Report
If you have received previous funding from RMHC NYTSA, you must include two copies of your previous
grant’s Follow-Up Report with this application. Simply indicating that you have submitted a Follow-Up
Report does not satisfy this requirement; the omission of a previous grant’s Follow-Up Report with this
application will result in your dismissal from grant consideration.

•
Checklist is found at the end of the Grant Application Form
Application must be typed, single-side pages only. Use standard black type that can be photocopied. Draw
all graphs, diagrams, tables and charts in black ink. Do not include any items that cannot be photocopied.
When submitting your request for funding, you must include two copies of the grant application and two
copies of each of the required items on the checklist.
IF ANY OF THE ITEMS REQUIRED IS NOT INCLUDED, YOUR APPLICATION WILL BE CONSIDERED
INCOMPLETE. You will be notified of incomplete applications.
