BE IT KNOWN that before me, the undersigned Notary Public, duly commissioned in and for the state and

parish aforesaid, and in the presence of the undersigned competent witnesses personally came and appeared:

hereinafter referred to as “Appearers” who declared that they are the owners of the following described

mobile home, to wit:

Said mobile home being situated on the following described immovable property owned by appearers, to

wit:

SEE EXHIBIT “A” ATTACHED TO AND MADE A PART OF THIS ACT

Appearers further declare that under the provisions of L.R.S. 9:1149.4, that mobile home described above

shall remain permanently attached to the tract of land described on Exhibit A and shall be hereinafter

considered as an immovable and as a part of said tract of land.

Appearers further declare that the title of manufacturer’s Certificate of Origin are voided unless above

mobile home is declared mobile with an Act of De-Immobilization at a future date.
