VEEU Fund Address Information

The following clarifies the purpose of each address that is required on the VEEU worksheet:

Section 1- Line 1b

Provides the complete mailing address of the electric distribution utility.

Section 4- Line 7

Provides the complete mailing address of the utility’s VEEU contact person. This address will be used for

future VEEU mailings, including statement/ bill, unless NECA the VEEUF Fiscal Agent is notified otherwise

by submission of the form below. If a separate entity is used to prepare the VEEU remittances, please also

provider preparer name and address.
