I/we understand that the address used to enroll my child at this school, which is I have listed

below, is the legal residence for my child.

I/we understand that it is Orange County Public Schools policy that if a parent, guardian, or

student falsifies any information regarding residency, the student-athlete will be ineligible from

representing any Orange County school for a period of one year from the date of discovery.

Furthermore, if a falsification is discovered, the student-athlete will be reported to the Florida

High School Athletic Association (FHSAA) who may impose further sanctions concerning

athletic eligibility.

When a student-athlete is determined to be ineligible due to falsification of residency, he/she may

appeal the ruling to the Florida High School Athletic Association.
