2Techbridge STEM Kit Reservation Form
•
Use this form to reserve Techbridge STEM (science, technology, engineering, and math) Kits or individual
activities. All kits are available on a first requested, first reserved basis. You will be notified if your choice is not
available.
•
No Walk-ins. Reservations may be made by phone, followed by a mailed or faxed copy of this form. Delivery
will be made to nearest Service Center for pick-up (many materials are too heavy to mail) and may take up to one
week. Kits must be reserved and paid for at least two (2) weeks in advance of pick-up date.
•
Include time to review the kit before meeting with the girls. STEM kits may be kept for up to 6 weeks from request
date.
•
Some of the great things about these kits are the career connections that are made for the girls. If you are renting
the entire kit, please be sure to offer the career activities. In the case of individual activities, an appropriate career
activity will be provided. Pre- and post-survey included in the kits must be administered to each participant and
the completed forms returned with the STEM kit. These will help us determine girls’ interest in STEM and will
inform future grant funding.
