AKC Foreign Registration Application

PLEASE READ INSTRUCTIONS BEFORE COMPLETING

GENERAL INSTRUCTIONS

The items listed below must be submitted for the AKC to consider registration:

The properly completed application, signed in ink.

The $50 registration fee, payable in U.S. funds — DO NOT SEND CASH.

A clear and legible photocopy of the original Certificate of Registration indicating the dog’s registry

and owner of record — DO NOT SEND THE ORIGINAL CERTIFICATE.

A clear and legible photocopy of the original foreign three-generation Certified Pedigree from the

registry in the dog’s country of birth (or Export Pedigree, where applicable) — DO NOT SEND THE

ORIGINAL PEDIGREE.

As of January 1, 1997, all foreign dogs being registered with the AKC are required to have positive

identification. This section of the application must be completed to show: a) the type of identifica-

tion, b) the identifying information and its location on the dog, and c) the registry recording the

identifying information.

If there are more than two (2) owners of the dog or more than two (2) breeders, a separate sheet

listing the complete names and addresses of each additional owner/breeder must be included.

A separate certification from each person or firm through whose hands the dog has passed,

whether as owner, importer, dealer, agent, on consignment, or otherwise, certifying that the dog

delivered by him/her is the same one described in the application and that the dog was delivered

by him/her directly to a named person or firm on a specified date (month/day/year). The first and

second transfers occurring in the USA should be entered in Sections IV and V of the application.

Additional transfers must be documented on Supplemental Transfer Statement forms, available

from the AKC.

If you are registering your dog and are not a resident of the United States, you need only complete

Sections I, II, VI, VII and VIII.

NOTE:

The dog must be registered with the name that appears on the Foreign Registration

Certificate/Certified Pedigree. Foreign titles will not appear on any AKC documentation.

IMPORTANT NOTICE

The American Kennel Club reserves the right to correct or revoke for cause

the registration of this dog. Any misrepresentation on this application is

cause for cancellation and may result in the loss of all AKC privileges.
